Your handshake tells a story. Large population studies — UK Biobank, the Prospective Urban Rural Epidemiology (PURE) study, and decades of gerontology research — have found that grip strength is one of the strongest physical predictors of how long and how well you will live. It outperforms blood pressure and even systolic blood pressure as a marker of all-cause mortality in older adults. The mechanism isn't magic: grip is a proxy for overall muscle mass, neuromuscular function, and the reserve your body has when illness or injury hits.
The encouraging part? Unlike your genetics or your childhood nutrition, grip strength is highly trainable. You can move the needle at any age. Here is the calm version of why it matters, how to measure it, and what to do about it.
Why grip strength predicts longevity
Grip strength reflects the integrity of the whole system: muscle quantity, muscle quality, nervous system drive, and the metabolic health that supports both. In the PURE study of nearly 140,000 adults across 17 countries, each 5kg decrement in grip strength was associated with a 16% higher risk of all-cause mortality, a 17% higher risk of cardiovascular death, and a 7% higher risk of non-cardiovascular death. The association held after adjusting for age, sex, education, physical activity, and comorbidities.
In the UK Biobank, low grip strength was linked to higher incidence of cardiovascular disease, respiratory disease, and cancer, as well as poorer cognitive outcomes and higher fracture risk. It is not that a weak hand causes these things — it is that a weak hand signals a system with less reserve. When you have less muscle to draw on, a hospital stay, a fall, or a bad infection takes a larger toll.
How to measure it (properly)
The gold standard is a hand-grip dynamometer (Jamar or a reliable digital equivalent). They cost £15–25 and last decades. Protocol matters:
- Sit or stand with your elbow at 90°, forearm neutral, wrist 0–30° extension.
- Take three trials on each hand, alternating, with 30–60 seconds rest.
- Record the maximum value (in kg) for each hand.
- Test at the same time of day; morning values tend to be slightly higher.
For men 50–59, population averages sit around 40–45kg; for women 50–59, around 25–28kg. By 70–79, those drop to roughly 30–35kg (men) and 18–22kg (women). But averages are not targets. The epidemiological signal is continuous: stronger is better, across the whole range. Your goal is to improve your own number, not to match a table.
No dynamometer? A bathroom scale works directionally: grip the sides, squeeze hard, read the kg. Less precise, but good enough to track trends. Or do a dead hang from a pull-up bar: 30–40 seconds is solid; under 15 seconds signals meaningful room to improve.
Three exercises that build usable grip
You do not need a gym full of specialised kit. These three cover the main grip patterns — crush, support, and pinch — and slot into any routine.
1. Farmer's carry (support grip)
Pick up a heavy dumbbell or kettlebell in each hand. Walk 30–40 metres with tall posture, shoulders down, core braced. Do 3–4 sets. Start with a weight you can hold for the full distance without your form breaking. Progress by adding distance, then load. This trains the fingers, forearm flexors, and the whole-body stabilisation that makes grip useful in real life — carrying shopping, luggage, a grandchild.
2. Dead hang (support grip + shoulder health)
Hang from a pull-up bar with a full grip (thumbs wrapped), shoulders active (pulled slightly down and back, not shrugged to ears). Build to 30–40 seconds for 3 sets. If that is easy, add a towel over the bar for a thicker grip challenge, or progress to single-arm hangs (alternating, 15–20 seconds each). The dead hang also decompresses the spine and maintains overhead shoulder range — a quiet two-for-one after fifty.
3. Plate pinch or towel row (pinch grip)
Pinch two weight plates (smooth sides out) between thumb and fingers. Hold for time — 20–30 seconds, 3 sets. No plates? Loop a towel through a kettlebell handle or around a dumbbell and row it, gripping the towel ends. Pinch grip is the pattern most neglected in standard training and the one that degrades fastest with age; it is also the one you use opening jars, turning keys, and holding a pen.
Programming: small dose, high frequency
Grip muscles recover quickly. Two to three short sessions a week (5–10 minutes) is enough to drive adaptation. Practical ways to fit it in:
- Finish your strength session with farmer's carries or dead hangs.
- Keep a Captains of Crush or similar gripper by the kettle; do a set while it boils.
- Do plate pinches while waiting for the microwave.
Consistency beats intensity. A little daily stimulus drives adaptation without frying your hands, wrists, or elbows. If you feel sharp elbow pain (medial or lateral epicondylitis), back off the volume and check your technique — often it is over-gripping with the wrist in extension.
What about the rest of the body?
Grip does not exist in isolation. The same studies that link grip to longevity also show that overall muscle mass and strength are protective. Grip is the canary; whole-body resistance training is the mine. If you are already following the Prime strength template (compound lifts, progressive overload, twice a week), your grip is getting trained indirectly. The direct work above simply ensures it is not the weak link.
For women especially: do not shy from heavy carries. The relative load (percentage of bodyweight) matters more than the absolute number on the dumbbell. A 50kg woman carrying 12kg per hand is moving 48% of her bodyweight — that is serious work.
A note on health conditions
If you have hand osteoarthritis, rheumatoid arthritis, carpal tunnel syndrome, or a recent wrist/elbow injury, check with your GP or a hand therapist before adding loaded grip work. Strengthening the right muscles can offload painful joints, but the wrong loading can flare them.
